DOCTOR / FELLOW must give us copies of :

1. Program Offer Letter or Match Verification Form
2. Completed and Signed Indemnity and salary Deduction Authorization Form ON APPROPRIATE FORM
3. Signed copy of Lease Guaranty Procedure accepting terms and conditions.

THEN SIGN 3 COPIES OF LEASE :

1. Fax first page to Partners Program coordinator with #1 and #2 and #3 above
2. send 3 copies of signed lease to Landlord IMMEDIATELY with GUARANTY EXPLANATION LETTER.

LANDLORD:
1. Wait for PARTNERS GUARANTY FORM from Partners Program Coordinator in mail
2. MAKE 3 COPIES OF GUARANTY LETTER
3. SIGN 3 COPIES OF LEASE
4. SEND COPY OF LEASE AND GUARANTY LETTER TO PARTNERS LEASING GUARANTY PROGRAM COORDINATOR
5. return 1 signed copy of lease to TENANT
6. keep 1 copy for landlord records
